This plan is administered by Anthem Student Insurance Plans and University Health Plans (UHP). Participation in this plan is mandatory for IU School of Medicine (IUSM), IU School of Dentistry (IUSD), IU School of Health and Human Sciences (SHHS), and IU School of Optometry (IUSO) students unless proof of comparable coverage and an enrollment waiver are submitted and approved by the waiver deadline. Plan Highlights

Eligibility and Enrollment

Participation in this plan is mandatory for IU School of Medicine (IUSM), IU School of Dentistry (IUSD), IU School of Health and Human Sciences (SHHS), and IU School of Optometry (IUSO) students who are currently taking classes. If you are an eligible student, you are automatically enrolled in the plan each semester and billed for the premium through your bursar account. Participation is mandatory unless proof of comparable coverage and a waiver form are submitted and approved by the deadline (see Premiums and Deadlines below). An email detailing the waiver process and any applicable deadlines is sent to eligible students each plan year.

Dependent Coverage

As an eligible student, you have the option to insure your dependents for an additional cost, including your spouse and unmarried children under the age of 26. Coverage is only available to your dependents if and when you are insured under the plan. Dependents must be enrolled by the deadlines listed in the tables below.

Enrollment can be completed online or by paper form. Regardless of your enrollment method, Anthem must receive your application and premium payment no later than the enrollment deadline listed below. Dependent premiums can be paid at the time of enrollment or by monthly installments; however, they cannot be charged to your bursar account.

Enroll Your Dependents

If the enrollment deadline has passed, you cannot enroll your dependents until the next coverage period, unless there has been a significant life change known as an IRS-qualifying life event (i.e., marriage, birth, loss of prior coverage). If enrollment is submitted within 30 days of the qualifying event, coverage will be backdated and begin on the date of the qualifying event. Enrollments submitted more than 30 days after the qualifying event will not be accepted, and you will have to wait until the next coverage period to enroll. Your medical coverage follows you wherever you go, including across the United States and overseas. As an Anthem member, you have access to care across the country through the BlueCard PPO program, and outside the U.S. through the Blue Cross Global Core program. Emergency Travel Services and AD&D Benefit

Your health plan coverage includes Academic Emergency Services (AES). With AES you have access to 24/7 assistance if you experience an emergency while traveling over 100 miles from home or outside your country. Services available include emergency medical evacuation and repatriation, pre-travel and safety assistance, and an accidental death & dismemberment (AD&D) benefit. Waiving Coverage

If you have existing health insurance coverage, you may be eligible to waive enrollment in the Professional Student Plan. To be eligible, your existing health insurance must be in place at the beginning of the semester/term and meet all of the following requirements: 

  • Provides coverage anywhere in the U.S. 
  • Provides emergency coverage internationally 
  • Provides coverage for pre-existing conditions
  • Provides essential health benefit coverage:
    • Ambulatory patient services;
    • Emergency services;
    • Hospitalization;
    • Maternity and newborn care;
    • Mental health and substance use disorder services;
    • Prescription drugs;
    • Rehabilitative and habilitative services and devices;
    • Laboratory services;
    • Preventive and wellness services and chronic disease management; and
    • Pediatric services, including oral and vision care. 
  • Has an annual deductible of $6,900 (individual) / $13,800 (family) or less
  • Has an Out-of-Pocket Maximum of $9,100 (individual) / $18,200 (family) or less
  • Has at least a $1,000,000 maximum benefit per lifetime per condition
  • Additionally, you must acknowledge that your health insurance coverage will remain in force for the entire academic year. If you involuntarily lose coverage prior to the end of the academic year, you agree to contact our office at  within 30 days of the date your coverage ends to enroll in the Professional Student Plan.  

The following types of plans do not qualify as eligible coverage:

  • Out-of-state HMO plans 
  • Fixed Indemnity plans
  • Short Term Insurance plans 
  • Emergency only coverage in Indiana plans
  • Medical share plans

The Healthy Indiana Plan (HIP) and Medicaid coverage will qualify as eligible coverage. However, these plans do not meet the full requirements regarding coverage outside of the state of Indiana, and abroad. Therefore, you may need to supplement when traveling.

How to Submit a Waiver

Fall Semester

Waivers for the fall semester can be submitted online. Have your current health insurance ID card and your IU student ID on hand, then follow these instructions.

  1. Go to the UHP Professional Student Insurance Plan website.
  2. On the left-hand side of the page select Waiver Form then choose your school (Medicine, Dentistry, Optometry, or Health & Human Sciences)
  3. Enter your date of birth (MM/DD/YYYY) and your 10-digit IU student ID number.
  4. Complete the form and enter your insurance information.

If your waiver is approved it will be effective for one academic year only. A new waiver must be completed each fall at the start of the new academic year. If you’re unable to access the waiver, it may be closed or you may not be currently eligible to complete the waiver.

Spring Semester

Waivers for the spring semester can be initiated by contacting IU Human Resources at , prior to the deadline date. You will be required to complete an enrollment form and submit copies of your insurance card and a summary of your medical benefits for review and approval.

The spring semester waiver deadline is February 28. After that, no waivers will be accepted for the spring enrollment period.

Frequently Asked Questions

As a student eligible for the Professional Student Plan, I will be turning 26 in the middle of the school year. Am I required to be on the plan for the full year or am I able to waive now and enroll when I lose my other coverage?

If you currently have outside coverage, you can waive coverage under the IU-sponsored plan at the beginning of the waiver period. When you lose your outside coverage, contact the student insurance office at . We will walk you through the process of prorating onto the Professional Student Plan from the date that you lose coverage.
