How to make a name and address change

It’s critical that the university has an accurate and up-to-date version of your home address, as well as your correct name, so that you continue to receive your mail from the university on time.

If you also need to change your school district or county on your W-4 tax record you can make those changes in the Employee Center through the One.IU portal.

Primary name change

To change your primary name, complete a Personal Data Change form and provide documentation of your new name to the IU Human Resources office. For U.S. employees, primary name changes must match the name on your social security card. Your primary (legal) name reflects how your name will appear on your paycheck, tax forms, and benefit plan documents.

Submit a personal data change form

  • Student, staff, and part-time employees: Submit to IU Human Resources at askhr@iu.edu or 2709 E. 10th Street, Ste 321, Bloomington, IN 47408.
  • IU Bloomington and Indianapolis academic appointments: Submit to school/department HR.
  • School of Medicine academic appointments: Submit to acadadmn@iu.edu.
  • All other academic appointments: Submit to the Campus Academic Affairs Office.
