Plan basics

Your vision coverage is through Anthem Blue View Vision and is included when you enroll in an IU-sponsored medical plan. You cannot enroll in vision coverage separately.

Vision services have a separate schedule of benefits, network providers, and costs. Expenses for vision services do not count toward your medical plan deductible, coinsurance, or out-of-pocket maximum.

Your vision benefits

  • Routine eye exams: You can get an eye exam every 12 months for a $10 copay.
  • Frames, lenses, and contacts: You have specific allowances and copays for in-network providers. Check out the 2025 Vision Benefit Summary for details.

Medical vs. routine eye care

Vision benefits cover routine eye care and vision correction. For medical eye treatments (for example, injuries, glaucoma, or retinal detachment), see a medical network eye care physician. These medical services will count toward your medical plan’s deductible and out-of-pocket maximums.

Find a provider

You have lots of in-network choices, including IU School of Optometry Eye Care Centers (Atwater Eye Care Center and Indianapolis Eye Care Center), LensCrafters, Pearle Vision, Sears, Target, 1-800 Contacts, other independent eye care professionals.

To find an in-network provider, follow these steps:

  1. Go to “Find Care” on anthem.com
  2. Log in to your Anthem account, or enter the prefix “IUC” in the Basic Search box and click “Continue”
  3. Enter your zip code or city and state
  4. Search for “Vision”

Now you can choose a specialty or browse a list of all network vision providers.

ID card: You don’t need a separate vision ID card. Simply show your Anthem medical ID card to your vision provider. You can find your card anytime, anywhere by logging in to anthem.com or the Sydney Health app.

When using an out-of-network provider

Using an out-of-network provider typically results in higher costs. You’ll need to submit a claim form to Anthem for reimbursement.
