This plan is administered by Anthem Student Insurance Plans and University Health Plans (UHP). Participation in this plan is mandatory for Indiana University International Students, Intensive English Program Students (IEP & PIE), and Visiting Scholars.
The International Student Health Plan offers medical, prescription, vision, and dental coverage for one premium. Review the plan documents and resources for details about your coverage including out-of-pocket costs, claims procedures, coverage while traveling nationwide or overseas, and exclusions.
Plan Highlights
- The plan year begins August 1 and ends July 31 the following year. Deductibles and out-of-pocket amounts restart annually on August 1
- Annual deductible of $500 single / $1,500 family for in-network coverage or $750 single/ $2,250 family for out-of-network coverage
- In-network preventive care covered at 100%
- IU Health Center copay of $15
- Office visit copay of $25 after deductible
- Urgent Care copay of $50 after deductible; Emergency Room copay of $100 after deductible
- Most other services covered with 20% coinsurance after deductible
- Includes therapy, behavioral health, mental health and substance use disorder services; See student brochure or plan booklet for more information
- Vision care coverage through Anthem Blue View Vision, including an annual eye exam with a $20 copay and discounts for vision wear
- Dental care coverage through Anthem Blue Cross and Blue Shield Dental Complete, including diagnostic and preventive services covered at 100% when using in-network providers

Student Eligibility & Enrollment
Participation in this plan is mandatory for Indiana University International Students, Intensive English Program Students (IEP & PIE), and Visiting Scholars. If you are an International or Intensive English student, you are automatically enrolled in the plan each semester and billed for the premium through your bursar account. For more information on enrollment, please contact your campus international student office. If you are a Visiting Scholar, you should contact your department or campus international student office for enrollment and payment information.
Dependent Eligibility & Enrollment
As an eligible student, you have the option to insure your spouse and unmarried children under age 26 for an additional cost. Coverage is only available to your dependents if and when you are insured under the plan. Enrollment can be completed online or by submitting a paper form to University Health Plans (UHP). Regardless of your enrollment method, UHP must receive your application and premium payment no later than the enrollment deadline listed in the next section. Dependent premiums can be paid in full at the time of enrollment or by monthly installments; however, they cannot be charged to your bursar account.
How to Enroll Your Dependents
- Visit the UHP Website.
- Select "International Students & Visiting Scholars”.
- Select “International Dependent Enrollment Form” from the left-hand menu, then your plan from the list.
- Enter the requested information and provide a copy of any required documentation.
If the enrollment deadline has passed, you cannot enroll your dependents until the next coverage period, unless there has been a significant life change known as an IRS-qualifying life event (i.e., marriage, birth, loss of prior coverage). In that case, you have 30 days from the date of the event to make changes to your plan. Coverage is then backdated to the loss of other insurance so that there is no break in coverage. Medical Coverage While Traveling
Your medical coverage follows you wherever you go, including nationwide and overseas. As an Anthem member, you have access to care across the country through the BlueCard PPO program, and outside the U.S. through the Blue Cross Global Core program. Emergency Travel Services and AD&D Benefit
Your health plan includes access to emergency travel assistance through Academic Emergency Services (AES). With AES you have access to 24/7 support if you experience an emergency while traveling over 100 miles from home or outside your country. Services include emergency medical evacuation and repatriation, pre-travel and safety assistance, and an accidental death & dismemberment (AD&D) benefit.
