Plan Benefits

Indiana University provides Basic Life Insurance to eligible enrollees. The amount of coverage is $20,000 for eligible enrollees under 65 (the benefit is reduced for those ages 65 and older). The university also provides Basic AD&D Insurance to eligible enrollees in an amount equal to the enrollee's Basic Life benefit. Accident Insurance benefits will reduce the same as the Life Insurance benefits. Indiana University pays the cost of this coverage. The policy number for Student Academic Appointees is 00241681-01. The policy number for Fellowship Recipients is 00241681-02. The policies are underwritten by Anthem Life Insurance.

Eligibility & Enrollment

Basic Life and Accidental Death & Dismemberment (AD&D) insurance is available to all eligible Student Academic Appointees and Fellowship Recipients enrolled in IU-sponsored medical and dental coverage Those who are eligible are automatically enrolled in the policy.

  • Student Academic Appointees are eligible on the day they become part of an eligible group (usually date of hire).
  • Fellowship Recipients on the day they become part of an eligible group (usually 8/1 for Fall recipients and 1/1 for Spring recipients).
  • There are no waiting periods or proof of good health requirements.
  • Enrollees must make a beneficiary designation by submitting the Beneficiary Designation Form to IU Human Resources.

Emergency Travel Assistance

Your Life and AD&D policy includes coverage under Academic Emergency Services (AES). With AES you have access to 24/7 assistance if you experience an emergency while traveling over 100 miles from home or outside your country. Services available include emergency medical evacuation and repatriation, pre-travel and safety assistance, and an accidental death & dismemberment (AD&D) benefit. Termination of Coverage

When coverage ceases under this group policy due to you terminating or ceasing to be in an eligible class for coverage, the Basic Life portion of the policy may be converted to an individual whole life insurance policy without providing proof of good health. Submit the Request for Life Insurance Conversion Information directly to Anthem for details.
