Enrollment

How do I enroll in IU Resident benefits?

Benefits enrollment information will be automatically sent to your IU email address once your hire date has been entered in IU’s system. You must enroll in most benefits within 30 days of your hire date, so make sure you complete online enrollment within that timeframe. If you miss the deadline, you must wait until the annual Open Enrollment period to enroll, with an effective date of January 1 of the following year.

How much will my benefits cost?

The monthly premiums for medical, dental, life, and disability insurance for you and your dependents (where applicable) are fully paid by the university or the hospital where you are completing your residency.

If you choose to enroll in the dependent care or healthcare flexible spending account, or supplemental AD&D insurance, or make contributions to an IU supplemental retirement plan, you must pay those premiums through payroll deductions.

Can I enroll myself or my dependents in benefits outside of the 30-day new hire enrollment period?

You can only enroll in benefits outside of the 30-day new hire enrollment period if you or your dependent experiences an IRS-qualifying life event (such as marriage, birth, loss of other coverage, or departure from/arrival to the U.S.). Enrollment changes must be made within 30 days of the event by submitting a Life Event change through the Employee Center.

Can I waive/terminate coverage for myself or my dependents at any time?

You can only waive/terminate coverage if you or your dependent experiences an IRS-qualifying life event (such as marriage, birth, loss of other coverage, or departure from the U.S.). Enrollment changes must be made within 30 days of the event by submitting a Life Event change through the Employee Center.

ID Cards

Where can I find my Anthem ID Card?

Once your enrollment in the plan has been processed, Anthem will mail a physical ID card to your home address on file. Cards are issued in the subscriber’s name only and should be used by all covered family members.

Cards can also be accessed 24/7 by logging in to Anthem’s Sydney Health app.

If you lose your physical ID card, you can print or request a new one be mailed to you by logging in to the Sydney Health app or by calling Anthem Customer Service at 844-736-0920.

When should I expect to receive my ID card?

ID cards will be sent to your home address within 10-14 business days from the date that you receive your benefits enrollment email notification from IU Human Resources.

I have not yet received my ID card, what should I do?

You can print or request a new ID card be mailed to you by logging in to the Sydney Health app or by calling Anthem Customer Service at 844-736-0920.

Health Care

What are preventive services?

Preventive services are routine healthcare services including physical exams, well-child visits, immunizations, lab tests (e.g., Pap, PSA, cholesterol), contraceptive services (e.g., IUD’s and sterilization), and other screening diagnostic services like mammograms and colonoscopies. You can view the list of preventive medical services or call Anthem Customer Service at 844-736-0920 for more information. Preventive services are covered at no cost when in-network providers are used.

How do I find a health care provider?

This plan uses the Anthem Blue Access PPO network in Indiana, the Anthem National PPO (BlueCard PPO) network in other states, and the Anthem Blue Cross Blue Shield Global Core network overseas. Once enrolled in the plan, you can find in-network providers by logging in to Anthem.com or the Sydney Health app and using the Find a Doctor tool.

What pharmacies can I use?

The CVS Caremark pharmacy network includes most retail chain pharmacies, such as CVS, Walmart, Target, and most supermarket and club pharmacy chains. Walgreens is not an in-network pharmacy. To locate network pharmacies or check prescription coverage and costs log in to Caremark.com or the Caremark app.

Does the plan include vision and vision wear coverage?

Yes. Vision and vision wear coverage is provided through Anthem Blue View Vision. This coverage is included with your medical plan enrollment, but vision services have their own schedule of benefits and network separate from medical benefits.
