Vision Wear Benefit

The vision benefit is included in the member’s enrollment in a medical plan, but the covered vision services have their own schedule of benefits and network providers separate from medical benefits. Additionally, the medical plan deductibles and co-insurance do not apply to vision benefits, and the amount the member pays for vision services do not accumulate toward the medical plan deductible or out-of-pocket maximums. 

In-network providers include the IU School of Optometry Eye Care Centers (Atwater Eye Care Center and Indianapolis Eye Care Center), LensCrafters, Pearle Vision, Sears, Target, 1-800 Contacts, and other independent eye care professionals. See the information specific to your health plan for information on how to find an in-network provider near you.

Benefits include:

The vision benefit is for routine eye care and corrective eye care only. For medical treatment of the eyes, visit a medical network eye care physician. Medical eye care includes services for such conditions as eye injuries, glaucoma, and retinal detachment. The medical deductible and out-of-pocket maximums apply to medical eye services.

Anthem Blue View Vision (Anthem PPO HDHP and PPO $500 plans)

Those enrolled in an Anthem PPO medical plan (Anthem PPO HDHP or Anthem PPO $500 Deductible), will receive the Anthem Blue View Vision benefit with no separate enrollment. Membership in this plan will be identified on the member’s Anthem medical plan card.

To find an independent in-network provider, follow the steps below:

  1. Visit www.anthem.com.
  2. Click on Find A Doctor under the “Useful Tools” section on the right side of the page.
  3. Either Log in if you already have an online account or enter as a Guest.
  4. Select Through my employer under the “How do you get insurance” question.
  5. Select Indiana under the “What state do you want to search in” question.
  6. Select Vision under “What type of care are you searching for” question.
  7. Select Blue View Vision under “Select a plan/network.”
  8. Click on the Continue button.
  9. Enter a Zip Code or City and State under the “Located near.” Select a distance to provider, if desired.
  10. Click the Search button.
EyeMed Vision (IU Health HDHP plan)

Those enrolled in the IU Health HDHP plan will receive the EyeMed Vision with no separate enrollment.  A separate membership card for EyeMed will be issued to members by mail.

To find an independent in-network provider, follow the steps below:

  1. Visit www.eyemed.com.
  2. Click on Find a Provider in the green menu bar at the top of the page.
  3. Enter your Zip Code to find providers nearest you.
  4. Select INSIGHT in the Choose Network drop down menu.
  5. Click the Get Results button.
  6. Or download the “EyeMed Members” app through the Apple iTunes store on your iPhone, iPad or iPod Touch.
