The plan is administered by Anthem Student Insurance Plans and University Health Plans. Participation in this plan is mandatory for IU School of Medicine (IUSM), IU School of Dentistry (IUSD), IU School of Health and Human Sciences (SHHS), and IU School of Optometry students unless proof of comparable coverage and an enrollment waiver are submitted and approved by the waiver deadline. Plan Documents
Eligibility and Enrollment
IU School of Medicine (IUSM), IU School of Dentistry (IUSD), IU School of Health and Human Sciences (SHHS), and IU School of Optometry students who are currently taking classes are automatically enrolled in this plan each semester, and are billed for the premium through their bursar account. Participation in this plan is mandatory for these professional students unless proof of comparable coverage and an enrollment waiver are submitted and approved by the waiver deadline. An email detailing the waiver process and any applicable deadlines is sent to eligible students each plan year.
How to Enroll Dependents
Eligible students may also insure their dependents, including their spouse and unmarried children under the age of 26, for an additional cost. Dependent eligibility is effective and expires concurrently with that of the insured student, meaning coverage is only available to dependents if and when the student is insured under the plan. Dependents must be enrolled for the coverage period by the enrollment deadline listed in the table below.
Enrollment can be completed online or by paper submission. Regardless of your enrollment method, Anthem must receive your application and premium payment no later than the enrollment deadline listed below. Dependent premiums can be paid at the time of enrollment or by monthly installments; however, dependent premiums cannot be charged to your bursar account.
If the enrollment deadline has passed, your dependents may not enroll until the next coverage period, unless there has been a significant life change known as a qualifying life event (i.e., marriage, birth, loss of prior coverage). If enrollment is submitted within 30 days of the qualifying event, coverage will be backdated and begin on the date of the qualifying event. If enrollment is submitted more than 30 days after the qualifying event, it will not be accepted and the student and their dependents will have to wait until the next coverage period to enroll. Travel and AD&D Insurance
Your coverage through your insurance plan includes Travel and Accidental Death and Dismemberment coverage. Academic Emergency Services (Travel) and AD&D coverage are underwritten by 4 Ever Life International Limited and administered by GeoBlue Worldwide. Your Travel coverage is through Academic Emergency Services and information about that can be found at the Academic Emergency Services Flyer & Medical Expense Claim Form. Frequently Asked Questions
As a student eligible for the Professional Student Plan, I will be turning 26 in the middle of the school year. Am I required to be on the Plan for the full year or am I able to waive now and enroll when I lose my other coverage?
All professional students who have current outside coverage are able to process through the waiver of the plan at the beginning of the waiver period. Then when you lose your outside coverage, contact the student insurance office at . We will walk you through the process of prorating onto the Professional Student Plan from the date that you lose coverage.
